Could you help me to make a form like this: kreativdentalclinic.eu/old/contact_mail.php in Joomla?
The difficult part is, that they have to be able to upload photos and in multiply format / jpg. ping, gif, ect. / and/or documents / Word, PDF, ect/

Is there any form panel like this out there?

Images part is flash object so you need to put something like that in some inset position.

Basically you can define fields as you like , and then program pages post request handling to threat all fields as you like.

There is no simple click-hear-and-there method to make that, we provide less options in our templates. Contact forms are coded manually by template providers.
