issue with template width

If you visit the page mesauam.com/alumni/alumni-database.html you will see that the form went beyond the page width. I think the issue is from the template. Pls how can I rectify it. Thank you

No table layout is like that. That's why table is considered bad element to use . Looking at your table it seams you will have to think of some other way to present that data perhaps list, or to pivot your table to two columns [property name]|[property value]
