i have a question to number 1. do you mean export? See screenshot.
2. Can you try deactivating it without any problems and then simply undo it and reactivate the extension?

Hello,
No, I didn't mean Export. Just open this page where Export and other tabs are and click SAVE AND CLOSE button.
Yes, you can try to de-activate extensions instead of uninstallation. Just make sure they are fully deactivated and don't have any effect to the website.
Regards,
Milos

I have now disabled all extensions
Unfortunately there is no improvement regarding the menu.
I also contacted German joomla support. There I was made aware of the following. Before I reinstall the template, I would like to ask you whether this could also be a reason why the menu does not work properly.
„With the first click on one of the submenu items, the menu item receives the class °FirstClick°. After that, the submenu can no longer be opened. Maybe support can do something with it.“
"3. If none of the above works, try re-downloading the template file "hot_clinic5x.zip" from our site and install it in Joomla. Before doing that, make a full website backup or backup."
is it okay with akeeba backup? shall i backup the Database too?

Hello,
The menu item receives the "FirstClick" class to allow opening the menu on mobile devices. The second click will go to the actual menu item page.
Full website backup using Akeeba will be enough. It includes the database also.
Download the template file from our site (don't just re-install the template that you downloaded a long time ago). Don't forget to open the template settings and save once after the template re-installation.
Regards,
Milos
