Contact Form

Hi all

Could you please help me with a problem I have found in the contact form? The thing is that I would like to add some fields to the form it self and i don know how or where to start. Like on the attached screen. Could you please point me to the right direction how would i achieve this?



I need to have 11 tick boxes and combo-box with 8 options. Both should be displayed in the received message.
