How i delete this /index.php/womens-health/ i dont want index.php and Category

You need to edit menu item alias, if menu item type does not have that parameter , change it temporally to some type that does , change, save than return back your menu item type.

i want to that. sitename.com/productname/ but your system is make that sitename.com/index.php/Categoryname/productname-detail

That is not part of template , that is how VM creates link, it think it has to do with alias in product details . You can install additional extensions to control urls
