Virtuemart upgrade breaks add address page was created by hkeyzer

Hi,

I've just upgraded a site from VM 2.0.20b to VM 2.0.22c, this causes the "add/edit billing address" page layout to become corrupted as per the attached image.

This does not occur with the add/edit shipping address page.

How do I correct this page?
