Hi Milos,
Thanks for the link, worked perfectly, however, the contact form is not appearing.
What do you mean by corrupted? Should I run some checks? Can you help me with this?

Hi Greg,

The form should be working demo.hotjoomlatemplates.com/fitness/index.php/contact but you most likely need to update the template files.

Download the template again and upload all template files by FTP. This way you will keep the template settings.
